Prerequisite Substition/Fulfillment Request

This form is for students who are requesting a course substitute or to determine if a course fulfills one of the listed LSU Vet Med DVM prerequisites. All courses must be completed at an accredited institution.

Complete the form below and then attach the syllabus for each course to an email with the subject “Prerequisite Substitution:

Last Name, First Name” (e.g., Prerequisite Substitution: Smith, Jane) to svmadmission@lsu.edu.
